Maybe you read about it while researching your symptoms at 1 a.m. Either way, it can feel like a confusing piece of a much bigger puzzle.

Here's the good news: insulin resistance isn't as mysterious as it sounds. It has a lot to do with one small but mighty organ, your pancreas. Understanding what's happening inside your body can help you feel more informed and less overwhelmed the next time this topic comes up.

What Is PCOS/PMOS, in Simple Terms?

Polycystic ovary syndrome, or PCOS, (recently renamed PMOS) is a hormonal condition that affects how the ovaries work. It's one of the most common hormone-related conditions in women of reproductive age. Researchers estimate it affects somewhere between 6% and 21% of women, depending on which diagnostic guidelines are used.

PCOS is usually identified by a combination of signs, which can include irregular periods, higher-than-typical levels of androgens (sometimes called "male hormones," though everyone has them), and small fluid-filled sacs on the ovaries that show up on an ultrasound.

But PCOS isn't only about reproductive health. It's increasingly understood as a whole-body, metabolic condition, and that's where the pancreas enters the picture.

Meet Your Pancreas: The Body's Blood Sugar Manager

Your pancreas is a small, leaf-shaped organ tucked behind your stomach. It has two main jobs: helping you digest food and helping regulate your blood sugar.

The blood-sugar part of that job falls to clusters of specialized cells called beta cells. Beta cells produce insulin, a hormone that acts like a key. When you eat carbohydrates, your blood sugar rises, and insulin "unlocks" your cells so they can absorb that sugar and use it for energy. Insulin also tells your liver and muscles to store extra sugar for later.

In a body without insulin resistance, this system runs quietly in the background. Blood sugar rises after a meal, insulin gets released, and things return to balance.

 

How Insulin Resistance Develops

Insulin resistance happens when the body's cells stop responding to insulin as effectively as they used to. The "key" doesn't fit the lock as well, so glucose has a harder time getting into the cells.

To compensate, the pancreas works harder. It pumps out more and more insulin to try to get the same result. This is called hyperinsulinemia, meaning higher-than-typical insulin levels in the blood. Over time, this extra demand can put ongoing strain on beta cells.

Insulin resistance itself doesn't always cause noticeable symptoms right away, which is part of why it can go undetected for a while. It's typically identified through blood tests ordered by a healthcare provider.

 

Why PCOS and Insulin Resistance Are Often Linked

Researchers have found that insulin resistance affects a large share of women with PCOS, some studies estimate more than half, and others put the number even higher, particularly among women with higher body weight. But insulin resistance can also occur in women with PCOS who are lean, so body weight alone doesn't explain the connection.

Scientists are still working out exactly why this link exists, but a few mechanisms are being studied closely:

Androgen levels and beta cells. Some research suggests that higher androgen exposure may affect how beta cells process and package insulin, potentially placing extra stress on these cells over time.

Signaling pathway changes. Studies have identified disruptions in the cellular pathways that normally help insulin "communicate" with muscle, fat, and liver tissue in women with PCOS, which may make it harder for those tissues to respond to insulin normally.

Inflammation. Chronic, low-grade inflammation is often observed in PCOS research, and inflammation is known to interfere with normal insulin signaling throughout the body.

A two-way relationship. Some researchers believe insulin resistance and PCOS may influence each other. Higher insulin levels may affect ovarian hormone production, while hormonal changes in PCOS may affect how tissues respond to insulin. This is an active area of ongoing research.

What This Might Look Like Day to Day

Because insulin resistance develops gradually, it doesn't always come with obvious warning signs. That said, some patterns researchers associate with insulin resistance include:

  • Patches of darkened, velvety skin, often on the neck, armpits, or other skin folds (called acanthosis nigricans)
  • Increased hunger or energy dips between meals
  • Difficulty losing weight despite consistent habits
  • Skin tags in areas of friction

These signs don't confirm insulin resistance on their own, and plenty of people with insulin resistance notice none of them. A healthcare provider can order specific lab work, such as fasting glucose, fasting insulin, or a HOMA-IR calculation, to get a clearer picture.

Factors Researchers Are Studying

Scientists continue to study which day-to-day factors are associated with insulin sensitivity in the general population. Areas of ongoing research include physical activity patterns, sleep duration and quality, dietary patterns rich in fiber and whole foods, and stress levels. None of these factors have been shown to reverse or cure PCOS or insulin resistance, and results vary widely from person to person. This is simply an active and evolving area of nutrition and endocrinology research.
