What Is PCOS/PMOS, Exactly?

Most people with NAFLD have no symptoms at all, which is part of why it's often called a "silent" condition. There are two general forms:
- Simple fatty liver (NAFL): Fat has built up in the liver, but there is little or no inflammation or liver damage.
- Nonalcoholic steatohepatitis (NASH): A more advanced form where the liver shows inflammation and damage in addition to fat buildup.
Because NAFLD often has no obvious symptoms, many people don't know they have it until it's found during a routine blood test or imaging for another reason.
Why Are PCOS and Fatty Liver Connected?
This is where the research gets interesting. Multiple studies have found that women with PCOS are more likely to have NAFLD than women without PCOS, even when body weight is similar between groups. Scientists believe a few shared underlying factors help explain this overlap:

Research suggests that women with PCOS who have higher androgen levels may have an increased likelihood of liver fat accumulation, independent of insulin resistance and body weight, though scientists note this relationship is still being studied and isn't fully settled.
Body fat distribution. Central, or abdominal, body fat is common in PCOS and is also strongly linked to fatty liver. Fat stored around the abdomen behaves differently than fat stored elsewhere and is more metabolically active, which can influence how the liver processes fat and sugar.
Metabolic syndrome overlap. PCOS and NAFLD often travel with other metabolic features, such as higher blood sugar, unfavorable cholesterol levels, and elevated blood pressure. Together, these are sometimes grouped under the term "metabolic syndrome," and researchers see this cluster of factors as a shared thread connecting the two conditions.
Why This Matters for Long-Term Wellness
Understanding this connection matters because both PCOS and NAFLD are linked to broader metabolic and cardiovascular health over the long term. Health researchers have noted that NAFLD is associated with increased risk for cardiovascular disease, and that this risk appears to be even more pronounced in people who also have PCOS.
This doesn't mean every woman with PCOS will develop fatty liver disease, or that everyone with fatty liver disease has PCOS. It simply means the two conditions share biological pathways worth understanding, especially since both can develop without noticeable symptoms in the early stages.
What Can You Do With This Information?
Because NAFLD often has no symptoms, awareness is the first step. If you have PCOS, it may be worth talking with your healthcare provider about routine liver health screening, especially if you have other risk factors like insulin resistance, higher body weight, or a family history of liver or metabolic conditions.
Lifestyle factors that support metabolic health more broadly, such as regular movement, balanced nutrition, and consistent sleep, are commonly studied in connection with both insulin sensitivity and liver fat levels. Research in this area is ongoing, and any changes to your diet, exercise routine, or supplement use should be discussed with a healthcare provider who understands your full health history.
